Preparing Applications

How to prepare and present my Application for Review to ADRC

Step 1. Obtain from ADRC an Application for Review form for your service type:
 
Tip: Either download from the link below or contact ADRC for a paper copy.
 
  • Each Application Form contains directions for preparing your Application for Review.
    Part A: Service Identification Information
    Part B: Argument and supporting documentation to support EACH quality Indicator you are requesting to be reviewed.
Step 2. Work through the Review process as a service to determine if you have sufficient grounds for a review.

  • Contact NCAC for clarification as to why the service did not achieve Accreditation status.
  • Decide whether the service has grounds for a review of the Accreditation Decision. Such grounds apply if you feel the service has sufficient evidence to prove that the practices within the service, at the time of the Validation Visit had been misunderstood, misinterpreted or misrepresented.

Step 3. Prepare and present your evidence for the quality Indicators you seek to have reviewed by ADRC.
Part A  

  • Part A of the Application form provides you with an opportunity to comment on aspects of the Validation Visit, Processes Verification, Validation Report, and Validation Evaluation Form.
  • Complete Part A once in your Application for Review.

    Tip: When determining if the service has sufficient grounds for review it is important to consider the Validation Report including the Validation Visit Processes Verification. The provision of a service representative’s signature against each of points in the Validation Visit Process Verification could be determined as evidence the relevant procedure occurred.

Part B  

  • Examine every quality Indicator assessed as not occurring within a Principle you request to have reviewed, to determine whether the service has sufficient evidence to present to ADRC for a review of that quality Indicator and Principle.
  • Repeat the completion of Part B for every quality Indicator you seek to have reviewed.
  • Ensure you have presented evidence for every Satisfactory quality Indicator assessed as ‘not occurring’, which you seek to have reviewed within a Principle.

    Tip: ADRC can only recommend an amendment to the assessment of a quality Indicator if ADRC is provided with sufficient evidence.

Step 4. Collate your documentation

  • Ensure any documentary evidence attached to your Application for Review is relevant and clearly identified as evidence for each of the quality Indicators you wish to be reviewed.

Step 5. Submit you Application for Review

  • Submit two (2) copies of your full Application for Review to ADRC within 20 days of receiving your Accreditation Decision.
